	Call for Programs: Weekly Rehearsals Description Fill Out BE SURE TO DOWNLOAD A PDF COPY OF ALL SUBMITTED REQUEST FORMS AT THE END OF THE SUBMISSION PROCESS, AS THIS IS THE ONLY WAY TO GET A COPY OF YOUR SUBMISSIONS FOR YOUR RECORDS. Dear Student Leader, Campus Activities' Scheduling Office is now accepting requests for weekly rehearsal reservations for the 2012/2013 academic year. Please take a moment to review the information below before submitting this "Call for Programs: Weekly Rehearsals" request form, as the process has changed from last year. When filling out this form, please be as flexible as possible; the more options you provide, the more likely it is that your request can be accommodated effectively. Submit one form per weekly rehearsal request; i.e., submit multiple forms if you'd like to rehearse more than once a week or in more than one room per evening. Submissions are due by noon on Friday May 4, 2012. We hope to notify groups of their assigned rooms/days/times via email by 6/1/12. AVAILABILITY -- Weekly rehearsals will be booked every week from 9/10/12 through 4/14/13, EXCLUDING holidays, breaks, and days unavailable due to previous bookings. -- Read your reservation notification emails carefully and completely when you receive them to understand exactly which dates you've been assigned and which dates have been ommitted from your weekly reservations. ADDITONAL REQUESTS -- Additional reservation requests will be handled on a first-come/first-served basis after the Call for Programs process is completed-- though we strongly advise you to take advantage of this Call for Programs process, as space will be extremely limited once it is completed. Student Organization Registration Quiz Description Fill Out As a requirement for registering, an organization must watch the presentations on the Campus Activities website and pass a short quiz. This quiz is designed to test your knowledge of information that was included in the online presentations. You must obtain at least a 70% on this questionnaire in order to "pass." Failure to obtain at 70% or greater will require you to retake the quiz and/or meet with your Program Manager. Please choose to fill out this quiz online. Printed PDF's will not be accepted. For questions regarding this quiz, or the registration process, please contact q.snellings@neu.edu Winter Activities Fair 2012 Registration Description Fill Out The Winter Activities Fair 2012 will take place on January 19th , from 2:30 - 4:30 PM. *Only currently active, fully registered* Northeastern student organizations will be able to take place in this year's Activities Fair! Registration will be open until all available tables have been filled. Please register early to guarantee your organization a spot! University departments and officially recognized student organizations can submit the following form to request reservations of the following major event venues for the Summer of 2012: Blackman Auditorium, Fenway Center, and the Curry Student Center Ballroom, McLeod Suites, and Fourth Floor Suites. Please note that the ground floor of the Curry Student Center is undergoing major renovation this summer; groups requesting space in the Curry Student Center this summer must therefore acknowledge that noise from this construction may distrub their meetings and events and that construction cannot be halted due to noise complaints before their requests can be considered. To learn about the capacities and capabilities of our major event venues, as well as their current availabilities, please visit our online scheduling service center. Faculty and staff members can log in to the myNEU portal and follow the "Meeting & Event Space Scheduling" link in the "Faculty / Staff Services" channel of the "Services and Links" tab. Calendar Listing Request Form (for non-student organizations only) Description Fill Out This form should be filled out by non-student organizations who wish to post their events to the Student Life/Campus Activities calendar. Requests submitted will be reviewed and approved within two business days. Student Organizations should log into the involveNU system to submit calendar requests for their events. Ticketing Request Form Description Fill Out Student organizations and campus offices should use this form when requesting ticketing services from the Ell Hall Box Office. Please make sure to submit this form at least ten (10) business days prior to your on-sale date to allow for complete set-up. Questions regarding this form or ticketing operations can be directed to Jeff Benham at j.benham@neu.edu. Membership Roster (proposed organizations only) Description Fill Out This form should be filled out by proposed organizations as part of their requirement for new group recognition with Campus Activities. Active and recognized organizations no longer need to complete this component as part of the bi-annual registration process. Instead, at least 12 active (non alumni) must be "joined" to your group within involveNU. Demonstration Permit Application Description Fill Out Please complete this form and submit to our office at least 7 days before your organization intends to demonstrate on campus property. Please review the demonstration policy that is found under the "policies" tab on the Campus Activities webpage. If your request for a demonstration is approved in accordance with the policy, you will be provided a permit that MUST be present on site with you. Failure to produce the permit when asked can result in disciplinary action and your removal from the site. Advisor Interest Form (full-time faculty/staff only) Description Fill Out Any full-time faculty or staff member at Northeastern is eligible to serve as an advisor to one of our student organizations. If you are interested in having your name kept on file should an organization need an advisor, please fill out this form and your information will be kept on file. Performer/Service Provider Pre-Contract Description Fill Out THIS IS NOT A CONTRACT Information that you input into this form will be used to generate a contract. Directions: Please complete this form using information that you should obtain from the facility representative. Be sure to schedule a meeting with your program advisor to review the details and logistics of your event. If the facility requires that you sign their own document, please attach those documents within this form. This form must be completed NO LATER THAN FOUR WEEKS prior to the event. Facility Pre-Contract Description Fill Out THIS IS NOT A CONTRACT Information that you input into this form will be used to generate a contract. Directions: Please complete this form using information that you should obtain from the facility representative. Be sure to schedule a meeting with your program advisor to review the details and logistics of your event. If the facility requires that you sign their own document, please attach those documents within this form. This form must be completed NO LATER THAN SIX WEEKS prior to the event. Travel Authorization Form Description Fill Out This form should be filled out anytime a student organization is sponsoring a OUTSIDE of the City of Boston. Individuals traveling outside of the Commonwealth of Massachusetts must also fill out the myTravel Plans (within the myNEU portal) in accordance with the following policy: Students participating in university sponsored travel outside of Massachusetts are required to have a completed entry in ’My Travel Plans’ for each trip. Failure to have a completed entry in ‘My Travel Plans’ is considered to be a violation of the ‘General Expectations’ section of the Student Code of Conduct under the heading Failure to Comply. (Contained in the Students’ Bill of Rights and Responsibilities section of the Student Handbook) Student Organization Fundraising and Cash Collection Request Form Description Fill Out Use this form when requesting permission to: host a table to raise money; collect cash; solicit donations/goods from corporations, etc. This form will be reviewed by the Assistant Director of Campus Activities. Please allow three weeks** for the approval process.

	FSL Community Service and Philanthropy Tracking Form Description Fill Out Purpose: To record all philanthropy and community service activities conducted, sponsored, and/or coordinated by your chapter. NOTE: Please complete this form no later than one week after the event. Fraternity and Sorority Grade Release Form Description Fill Out Fraternity and Sorority Grade Release Form By fulling out this form, I authorize the disclosure of my semester and cumulative grade point averages to my chapter president, scholarship officer, chapter advisors, faculty advisor, (Inter)National Organization and the Student Activites and Leadership staff so that awards, recognition, and educational needs can be determined as long as I am affiliated with the chapter.

	OUT@NU Speakers Bureau Application Form Description Fill Out APPLICATIONS ARE DUE BY FRIDAY, JANUARY 13, 2012 AT NOON. About the Speaker's Bureau OUT@NU Is a program that has been developed to educate the Northeastern Community about the LGBTQ community. Our goal is to educate people on the current realities and personal histories of those who identify as Lesbian, Gay, Bisexual, Transgender, Queer, Questioning, and related identities. Through personal story-telling or narrative, speakers share their experiences and growth, thereby promoting knowledge, compassion, and understanding about LGBTQ people, issues and concerns. In our traditional format, a panel of 4-5 speakers each share their personal stories, illustrating how LGBTQ lives and experiences are diverse, complex, and richer than portrayed in mainstream media. The Q&A component of our panel encourages broader, deeper dialogues about sexual orientation, gender identity and expression, and possibilities for a better world. About the Training The training focuses on the importance of narrative in combating oppression, storytelling techniques, the mechanics of the Speakers Bureau program, and how to handle difficult questions. Any individuals of any sexual orientation or gender identity are welcome to apply, so long as they are willing to share their personal coming out narrative. *Training Date* Sat, January 21, 2012, 10:00 AM - 5:00 PM The training location will be shared with members as we get closer to the training date. Food and snacks are provided! The entire training must be attended in order to participate in the Speakers Bureau. Expectations of the Speakers Bureau Members Only speak from your own life experiences, and do not generalize your experiences to others of your social identity or in the LGBTQ community. Only share your story, and answer questions in a non-confrontational manner - do not lecture or debate. Be willing to give and accept feedback. Belief in and commitment to social justice. Consistent access and use of email. *Time Committment* Pre-Training: Complete and electronically submit the accompanying Speakers Bureau Application One-Time Training: Attend and participate in the entire training - dates and times listed above Post-Training: Two panels per semester, scheduled at your convenience according to your availability (panels are usually 60-90 minutes in duration) *Qualifications* Comfort with your own sexual orientation and gender identity and expression A strong desire and ability to educate others about lesbian, gay, bisexual, transgender, queer, questioning, and related identity issues A willingness to speak personally and publicly about issues of sexual orientation and gender identity and expression as they relate to your own life Commitment to being an ally to folks of social identity groups other than your own Ability to meet the time commitment and expectations of all Speakers Bureau members.
